The SPX Athletic Department requires all athletes to complete a set of participation forms. The complete set must be on file before playing or trying out for any sport at St. Pius X High School.
Submit Forms Online
All required forms can be accessed and completed online in our user-friendly Athletics Parent Portal through the Rank One Sport website. Rank One Sport is a software that allows us to manage our athletic paperwork and injuries online. The portal will allow you to automatically track the status of complete and incomplete forms in one convenient location. This will help you turn in forms quicker and help us be more efficient and environmentally sound by saving paper.
- If there is any problems with submitting forms or insurance cards on Rank One, please contact the Athletic Trainer, Mr. Jeremy Kallus, at kallusj@stpiusx.org.
Forms to Print for Physician
