To apply by fax or mail:   
After printing and completing the application from the link below, you may fax it to 713-692-5725 or mail it to the following address:

If applying by fax, the $35 application fee will need to be mailed before your application can be processed.  If applying by mail, please include the application fee in the same envelope as the application.
